'Green herb + chemical fluid' — how much of it is real?'

① It all starts from something real모든 건 실제로부터 출발한다

The idea of pouring a liquid on a wound to stop bleeding and disinfect it at once is not pure fiction. Kaolin haemostatic gauzes such as QuikClot, used in field first aid, speed up clotting by rapidly absorbing water from the blood and so concentrating blood cells and clotting factors at the wound. Antiseptics such as povidone-iodine, meanwhile, combine low cytotoxicity with broad antimicrobial activity and are still recommended for acute and chronic wound care. So the "herb (a natural component) plus chemical fluid (a synthetic antiseptic and haemostat)" story can be read as a compressed metaphor for the two pillars of real first aid: stopping bleeding and disinfecting.

A family of combat and emergency haemostatic gauzes that use kaolin to accelerate the blood's own clotting. They are used to bring bleeding under control quickly where ordinary pressure cannot stop it, as in severe trauma or arterial bleeding.

② And then past the real, into invention실제를 넘어선 상상의 세계로

The problem is what comes next. Grant the bleeding and the disinfection; tissue regenerating in seconds and a cut sealing itself instantly is beyond current medicine. Real replantation of a severed limb is microsurgery, suturing vessels, nerves, bone and tendon one by one under a microscope. The operation is long and complex, and papers on replantation rehabilitation agree that the rehabilitation afterwards matters as much as the surgery. Health filling like a gauge is just as far from reality. Wound healing is a biological process through inflammatory, proliferative and remodelling phases, taking at minimum days and often weeks, and no drug compresses that process into seconds. Resident Evil's healing items, in the end, borrow the logic of real first aid — haemostasis and antisepsis — and lay pure fantasy on top of it: instantaneous, complete regeneration.

Why PUBG split its healing items in two배틀그라운드는 왜 회복 아이템을 두 갈래로 나눴을까

PUBG's healing items divide broadly in two. Bandages restore health over time up to 75 HP; first aid kits and med kits take HP to 75 and 100 respectively on completion. Painkillers, adrenaline syringes and energy drinks, by contrast, fill a separate resource called the boost gauge. While that gauge holds a charge, health recovers gradually over time, and at higher levels movement speed briefly increases a little as well.

Direct HP recoveryHP

Boost gauge

HP 10 / 100
Boost 0 / 100

You start at 10 HP. Only one item can be used at a time; boost decays over time and slowly restores HP.

That division lines up roughly with a real division in pharmacology. Treatments that intervene at the wound itself, like a bandage or a first aid kit, are close to what is actually called local treatment — handling the damaged tissue directly. Painkillers, adrenaline and caffeine (the active component of an energy drink), on the other hand, are none of them substances that heal a wound. A painkiller of the NSAID class is a drug that blunts the pain signal itself by inhibiting prostaglandin synthesis, not one that regenerates damaged tissue. Adrenaline (epinephrine) is a hormone that raises cardiac output and blood flow to skeletal muscle, flipping the body momentarily into "fight mode", and caffeine acts at adenosine receptors to lower the rating of perceived exertion during exercise, delaying the feeling of tiring rather than the tiring. All three share only this: they temporarily manipulate the body's perception and performance. None repairs damaged tissue. This is where the game's boost gauge quietly filling health over time parts company with reality — because neither a painkiller, nor adrenaline, nor caffeine puts back lost blood or lost tissue.

Ana in Overwatch fires a "sleep dart" as her secondary fire. A target hit by it falls asleep on the spot as the screen changes — land the shot and consciousness goes almost simultaneously.

The premise of injecting a drug by dart to put someone to sleep is not fiction. Wildlife medicine calls it remote drug delivery and really does use it for anaesthesia and capture. In practice, though, the effect arrives not at the moment of the hit but after the drug has been absorbed. A study darting wild bighorn sheep in the Canadian Rockies reported a mean of 7.7 minutes (range 5.8–9.7) to full anaesthesia with a combination of alfaxalone, medetomidine and azaperone. During those minutes the animal can still move, and may be injured or escape — which is why shortening that window is a research subject in its own right. A study in polar bears reported that adding hyaluronidase, an enzyme that helps a drug spread through tissue, cut mean induction time from 15 minutes to 10.

What the game's sleep dart compresses is not what an anaesthetic does but the rhythm of a shooter, which needs a threat neutralised at once. The concern of people who actually work with darts is closer to the opposite — not how fast the animal goes down, but how safely the few minutes before it does can be got through.

② Lúcio's "turn it up!" — the pain does drop, but not for the reason you think

Lúcio carries no weapon and no potion. Simply by playing certain music through the speakers on his skates, he continuously restores the health of nearby allies.

"Music reduces pain" is not a claim without evidence. According to a systematic review by Hole and colleagues, published in the Lancet in 2015, pooling 73 randomised controlled trials of perioperative music, patients who listened to music reported less pain (standardised mean difference −0.77) and less anxiety (−0.68), used less analgesia (−0.37) and were more satisfied than those who did not — and the effect held even under general anaesthesia. Length of stay, however, did not fall.

So does music reduce pain by releasing endorphins — endogenous opioids — as is often said? A 2022 study by Lunde and colleagues is cautious about that account. Forty-eight healthy participants were given heat-stimulus pain while listening to music, natural sound or white noise, each under an opioid blocker (naltrexone) or a dopamine blocker (haloperidol); neither abolished music's analgesic effect. What did track the pain actually felt, more closely, was how much a participant had expected in advance that the sound would help — on this experiment alone, music's analgesia is better explained by an expectation mechanism close to placebo than by opioid or dopamine pathways.

The closest thing to what Lúcio's speakers really do is not magic that heals a wound, but a device that makes a patient believe this will help. The game amplifies that effect and draws it as an actual treatment; real music regenerates no tissue — it only helps pain be felt as less painful.

League of Legends' healing abilities — how much of it is real?

① Dr. Mundo's ultimate — why a real transfusion takes minutes

Dr. Mundo in League of Legends is, from the outset, a character who took over an asylum and has injected himself with every drug going. His ultimate, Sadism, plants an enormous syringe into himself and returns a large share of his lost health in seconds.

Rapidly transfusing blood or fluid into a massively bleeding patient to restore circulation is a real intervention, but "one injection equals instant recovery" it is not. A study of trauma data from 132 hospitals across Japan reported that the time from arrival to first transfusion varied from a mean of 4 minutes to 58 minutes between hospitals, and that each minute of delay in starting transfusion was associated with significantly higher in-hospital mortality (adjusted odds ratio 1.015). Health does not fill the instant the syringe goes in; rather, how far those minutes can be cut is what separates living from dying.

There is a more counter-intuitive fact still. Real trauma resuscitation uses a strategy of permissive hypotension, deliberately not forcing blood pressure back to normal — because raising it too quickly can burst a clot that has only just formed and restart the bleeding. A systematic review pooling 11 studies and 4,529 patients reported in-hospital mortality of 6.3% in the permissive-hypotension group against 16.3% for conventional fluid resuscitation. What Mundo's ultimate compresses is not what a transfusion does, but the momentum of a game where a health bar fills instantly — for real clinicians, judging precisely when to stop and when to give matters more than how much is given.

② Renata's Q — why an inhaled drug acts so fast

Renata Glasc, the Chem-Baron of Zaun, throws a grenade of dispersing chemical gas with her Q, Handshake. Allies touched by the gas recover health and enemies take damage — the effect appearing the moment it is breathed in.

Delivering a drug by inhalation genuinely is the textbook route when speed of onset is what you want. A study of a technique that vaporises pure drug particles with heat for deep inhalation reported that giving the anxiolytic alprazolam this way took just 20 seconds to reach peak blood concentration — faster even than a five-second intravenous injection. The lung is this fast because of its structure. According to a review of systemic drug delivery through the lung, the alveoli present an enormous surface area in direct contact with blood vessels, so a lipophilic drug crosses the alveolar membrane into the bloodstream almost immediately — a far shorter route than an oral drug swallowed through the gut.

That speed of absorption is a double-edged thing, however. As fast as the inhaled route is, it is hard to control where in the lung the drug lands and how much of it does, and variation between individuals is large — unlike Renata's gas, which cleanly separates ally from enemy to heal or harm, a real inhaled drug is hard to control even to the level of who breathed in how much.

When games and reality borrow from each other게임과 현실이 서로를 빌려 쓸 때

The healing items looked at here are all, in the end, the result of games taking the real body and real medicine and compressing them to their own rhythm. There are moments when the arrow points the other way. The headshot is the obvious example: the rule that hitting the head does critical damage is close to a game borrowing the fact that damage to the brain is more dangerous than damage anywhere else. And now, in reverse, the word "headshot" itself — meaning a precise, decisive single blow — has flowed back into everyday speech with nothing to do with guns. Once a game borrows the rules of the body to build its weapons and its healing items, the language and the instincts it makes flow back into reality: a loop.
