It is hard to picture now, but in Western societies flesh was for a long time a sign of wealth and of health. Well into the late nineteenth century a full body was taken as evidence of resistance to disease and of a comfortable life.

Then, through the later nineteenth century, trams and motor cars reduced physical activity while the food supply grew abundant, and society began worrying about being overweight for the first time. Around 1900 a cookery book defined excess flesh as a kind of unwelcome disease, and from this point the diet industry began in earnest.

The English Romantic poet George Gordon, Lord Byron, was famously terrified of putting on weight. When in 1818 he gained enough to look pale and swollen, he threw himself into an extreme regimen of drinking vinegar mixed with water.

This "vinegar diet" later became fashionable as a beauty practice among Victorian women. Byron cycled between severe restriction and bingeing, and historians consider that this contributed to his death at 36. The habit of regularly drinking vinegar carries a series of harmful health effects.

The first "modern diet"최초의 '근대 다이어트'

What is usually named as the first popular diet in history is Letter on Corpulence, a pamphlet published in 1863 by the English undertaker William Banting. Having gained enough weight that his hearing and eyesight suffered, he followed the prescription of the physician William Harvey: cut out bread, butter, milk, sugar, beer and potatoes, and eat mainly beef, mutton and fish. Within a year he had lost 21 kg (about 46 pounds) and roughly 31 cm from his waist.

The pamphlet made such an impression that in English "to bant" came to be used as a verb meaning "to diet".

Keeping a parasite inside the body몸속에 기생충을 기르다

One of the most notorious methods is the "tapeworm diet". Swallow a pill containing tapeworm eggs or larvae, let the parasite take up residence, and it would intercept your nutrition so that you could eat freely and still lose weight — that was the logic.

가장 악명 높은 다이어트법 중 하나가 '촌충 다이어트'다. 촌충 알이나 유충이 든 알약을 삼켜 몸속에 기생시키면, 촌충이 영양분을 가로채 마음껏 먹어도 살이 빠진다는 논리였다.

How many people actually took a real tapeworm pill, though, is disputed among historians. A good proportion were probably frauds selling a placebo. Deaths during the removal of a tapeworm, on the other hand, are genuinely on record. The US FDA prohibits it today, but the idea keeps resurfacing.

다만 실제로 얼마나 많은 사람이 진짜 촌충 알약을 복용했는지는 역사가들 사이에서도 의견이 갈린다. 상당수는 위약을 판매한 사기극이었을 가능성이 크다. 반면 촌충을 제거하는 과정에서 목숨을 잃은 사례는 실제로 기록돼 있다. 현재 미국 FDA가 금지하고 있지만 온라인 암시장에서는 근절되지 않았다.

"Chew and the weight comes off""씹으면 살이 빠진다"

The American businessman Horace Fletcher, refused life insurance at 40 weighing about 98 kg, devised a method of chewing food until it liquefied before swallowing. Believing digestion happened mostly in the mouth, he is said to have chewed a piece of shallot 722 times, over about twelve minutes.

미국의 사업가 호레이스 플레처는 40세에 약 98kg의 몸으로 생명보험 가입을 거절당하자, 음식을 액체가 될 때까지 씹어 삼키는 식이법을 개발했다. 소화가 대부분 입에서 이루어진다고 믿었던 그는 파 한 조각을 722번, 약 12분 동안 씹은 것으로 알려져 있다.

Losing about 19 kg this way, he was hailed as "the Great Masticator" and became a sensation. John D. Rockefeller, Arthur Conan Doyle and Upton Sinclair were fervent adherents, and Herbert Hoover made him an official food advisor to the First World War relief effort.

Step onto the platform and an electric motor on a waist-high column shakes the belt. The user does nothing. Just stand there, and the vibration would supposedly break down the fat around your middle. That was the promise.

Its roots were medical equipment. In the mid-nineteenth century the Swedish physician Gustav Zander devised devices combining exercise with mechanical vibration, laying the foundation of mechanotherapy; in 1895 John Harvey Kellogg, of cereal fame, built whole-body vibration machines on the hypothesis that vibration aided circulation and relieved constipation.

Moving from there to a weight-loss product did not take long. The belt vibrator and its sales material held by the Kansas Historical Society show advertising promising to reduce specific measurements in fifteen minutes a day, while you watched television or napped. There was one wave in the 1900s–1930s and another in the 1950s–1960s.

What is interesting is the manufacturers' own posture. A Walton product in the same collection carried a disclaimer that it was not for weight loss. Even the sellers were not confident of the effect.

What the evidence says근거는 무엇을 말하나

What this machine sold was the idea of spot reduction: that stimulating a particular area removes the fat from it. It does not hold even for real exercise, never mind vibration.

이 기계가 판 것은 '부분 감량(spot reduction)'이라는 관념이다. 특정 부위를 자극하면 그 부위의 지방이 빠진다는 생각인데, 진동이 아니라 실제 운동으로도 성립하지 않는다.

In a randomised controlled study in which 24 healthy adults did seven abdominal exercises five days a week for six weeks, neither abdominal subcutaneous fat, nor body fat percentage, nor waist circumference fell significantly. Only abdominal endurance improved.

In the Jazz Age, as the thin, androgynous "flapper" shape came into fashion, young women took up smoking as a means of dieting alongside extreme restriction to celery, tomatoes and lettuce.

In 1928 the tobacco company Lucky Strike ran a large advertising campaign under the slogan "Reach for a Lucky instead of a sweet". The advertising presented the cigarette as a substitute for confectionery — that is, as an appetite suppressant, with famous actors and singers appearing in it.

The account that this campaign normalised smoking among young women has evidence behind it. In an analysis of 165,876 people from US national surveys covering 1910–1977, the rate at which adolescent girls began smoking rose sharply in exactly the years the campaign ran.

What the evidence says근거는 무엇을 말하나

Nicotine's appetite-suppressing effect is real enough. The problem is the other side of the scale. In a study following some 34,000 British doctors over 50 years, lifelong smokers died on average ten years earlier than non-smokers — and stopping before 30 avoided almost all of that excess risk.

2,4-Dinitrophenol (DNP) came to notice through weight loss among workers in explosives and dye factories. In the 1930s it was prescribed as a weight-loss agent, exploiting the way it disrupts cellular energy production and generates heat.

2,4-디니트로페놀(DNP)은 폭약과 염료를 만들던 공장에서 노동자의 체중이 빠지는 현상으로 알려졌다. 1930년대에는 이 물질이 세포의 에너지 생산을 흐트러뜨려 열을 내는 원리를 이용해 체중 감량제로 처방됐다.

The problem was that the faster the metabolism, the harder body temperature was to control. Cataracts, liver and kidney damage, hyperthermia and deaths followed, and the US FDA classified DNP in 1938 as extremely dangerous for human use. It still circulates online as a supplement today; that does not make an old drug safe again.

Grapefruit, soap, and the reducing salon자몽, 비누, 그리고 리듀싱 살롱

From the early 1900s came a flood of products: reducing soap (a dollar a bar), laxative-laden "Bile Beans". In the 1930s the "grapefruit diet" spread on the baseless belief that eating grapefruit at every meal activated a fat-burning enzyme.

1900년대 초부터 살 빼는 비누(개당 1달러), 하제가 든 '바일 빈스' 같은 상품이 쏟아졌다. 1930년대에는 매 끼니 자몽을 먹으면 지방을 태우는 효소가 활성화된다는 근거 없는 믿음에서 '자몽 다이어트'가 유행했다.

Through the 1940s and 1950s, "reducing salons" equipped with the vibrating belts above and other motorised rollers did good business, and the cabbage soup diet appeared in the same period.

The "Sleeping Beauty diet" started from the simple idea that you cannot eat while asleep. It meant taking sedatives to sleep 14 to 20 hours a day, cutting off eating at the source.

"자고 있으면 먹을 수 없다"는 단순한 발상에서 출발한 것이 '잠자는 숲속의 공주 다이어트'다. 진정제를 복용해 하루 14~20시간을 잠들어 지내며 식사를 원천 차단하는 방식으로, 1966년 재클린 수잔의 소설 『밸리 오브 더 돌스』에서 인물들이 스위스의 '수면 클리닉'을 찾는 장면으로 대중에 알려졌다. 제목의 '돌스'는 당시의 진정제와 각성제, 곧 바르비투르산계 약물과 암페타민을 가리키는 은어였다. 최근에도 친거식증 온라인 커뮤니티를 중심으로 다시 회자된다.

What this method demands is written on the drugs themselves. For benzodiazepines — the sedatives that replaced barbiturates and are the most widely used today — the US FDA in 2020 required a boxed warning covering abuse, addiction, physical dependence and withdrawal.

Atkins: a prescription from a century before, revived앳킨스, 100년 전 처방의 부활

In 1972 the cardiologist Robert Atkins published Dr. Atkins' Diet Revolution, reviving Banting's low-carbohydrate principle in modern form. Across its editions the book sold more than 15 million copies.

Weight Watchers, launched in 1963, put "managing eating habits" ahead of "controlling the diet" and turned dieting into a community activity; through the 1980s to the 2000s the Beverly Hills, Mediterranean, Zone, South Beach and paleo diets competed for attention.

The Last Chance Diet, published in 1976, popularised a very-low-calorie regimen of drinking liquid protein instead of solid food. The products of the time were sold as though they could be used without medical supervision, while their composition and management varied widely.

1976년 출간된 『The Last Chance Diet』는 고형식 대신 액체 단백질을 마시는 초저열량 식단을 대중화했다. 당시 제품은 의료 감독 없이도 사용할 수 있는 것처럼 팔렸지만, 식단의 성분과 관리 방식은 제각각이었다.

From 1977, cardiac rhythm abnormalities and sudden deaths were reported. A study published in 1979 investigated the sudden deaths of 17 people on liquid protein diets and pointed to a link between prolonged extreme restriction and a prolonged QT interval. Later protein-sparing modified fasts are designed under clinical supervision, and cannot be read as the same thing as the fad products of that period.

Fibre imitating fullness포만감을 흉내 낸 섬유질

The cotton ball diet is an internet-born fashion for swallowing cotton soaked in juice to fill the stomach. It spread with claims of being standard practice in modelling, but survey data establishing its scale is hard to find. What is established is not the extent of the craze but the danger of the behaviour.

솜뭉치 다이어트는 주스에 적신 솜을 삼켜 배를 채운다는 인터넷발 유행이다. 모델 업계의 표준 식단이었다는 식으로 퍼졌지만, 그 규모를 입증하는 조사 자료는 찾기 어렵다. 확인되는 것은 유행의 존재보다 행동의 위험성이다.

Cotton is not food and provides no nutrition. Both ordinary cotton and synthetic fibre can ball up and obstruct the gastrointestinal tract or cause malnutrition. The Center for Young Women's Health at Boston Children's Hospital warns that the behaviour can lead to bowel obstruction and surgery.

솜은 음식이 아니어서 영양을 제공하지 않는다. 일반 솜과 합성 섬유 모두 덩어리로 뭉쳐 위장관을 막거나 영양실조를 일으킬 수 있다. 보스턴소아병원 청소년건강센터는 이 행동이 장폐색과 수술로 이어질 수 있다고 설명한다. 이 사례는 '다이어트법'이라기보다 섭식장애와 연결해 다뤄야 할 위험 신호에 가깝다.

The claim that air and sunlight are enough to live on공기와 햇빛만으로 살 수 있다는 주장

Breatharianism is the claim that a person can live on air, sunlight and spiritual energy instead of food and water. It has been presented as a form of weight loss, but it is less a kind of diet than a claim denying a physiological limit.

브레서리언(breatharian)은 음식과 물 대신 공기, 햇빛, 영적 에너지로 살아갈 수 있다는 주장이다. 체중 감량법의 형태로 소개되기도 했지만, 이는 식단의 한 종류가 아니라 생리학적 한계를 부정하는 주장에 가깝다.

In a review of investigated cases of the claim, none could be taken as having withstood prolonged food and water restriction under controlled conditions. Cut out food and water and dehydration and electrolyte disturbance become the problem long before weight does. Cutting out food and water is not weight management; it produces dehydration and nutritional deficiency.

이 주장을 검증한 사례들을 검토한 연구에서, 장기간의 음식·수분 제한을 통제된 조건에서 견뎠다고 볼 수 있는 사례는 확인되지 않았다. 음식과 물을 끊으면 체중보다 먼저 탈수와 전해질 이상이 문제가 된다. 음식과 물을 끊는 방식은 체중 관리가 아니라 탈수와 영양 결핍을 일으킬 수 있다.

Tea and charcoal promising a detox해독을 약속한 차와 숯

Through the 2010s, "teatoxing" — drinking a detox tea for a set period — spread quickly on social media. These products, containing diuretics or laxatives and caffeine, advertised reduced bloating and weight, but no clinical evidence of removing toxins or reducing body fat has been established.

2010년대 소셜미디어에서는 일정 기간 '디톡스' 차를 마시는 티톡스가 빠르게 퍼졌다. 이 제품들은 이뇨제나 하제, 카페인을 넣어 붓기와 체중을 줄인다고 홍보했지만, 독소를 제거하거나 체지방을 줄인다는 임상 근거는 확인되지 않았다.

The charcoal diet, taking activated charcoal, made a similar promise. Activated charcoal has medical uses in certain poisonings, but it is not an everyday weight-loss agent. It can interfere with the absorption of other drugs, so the phrase "natural detox" is not enough to judge its safety.

활성탄을 먹는 차콜 다이어트도 비슷한 약속을 내세웠다. 활성탄은 일부 중독 상황에서 의료적으로 쓰이지만, 일상적인 체중 감량제는 아니다. 다른 약물의 흡수를 방해할 수 있어 '천연 해독제'라는 말만으로 안전성을 판단할 수 없다.

In 2016, under the influence of a television documentary, a low-carb high-fat diet — 70–75% fat, 5–10% carbohydrate — became popular enough to cause butter shortages in the large supermarkets. In recent years "16:8 intermittent fasting", sixteen hours fasting and an eight-hour eating window, has settled in as the most common approach.

What the evidence says근거는 무엇을 말하나

Evidence that intermittent fasting beats ordinary calorie restriction at the same intake is weak. A network meta-analysis pooling 167 randomised controlled trials (11,998 people) concluded that the effect ultimately depends on how much you cut, and when you eat makes little difference.

간헐적 단식이 같은 칼로리를 먹는 일반적인 식단 제한보다 더 효과적이라는 근거는 약하다. 167건의 무작위 대조 시험(1만 1,998명)을 종합한 네트워크 메타분석은, 감량 효과가 결국 얼마나 줄이느냐에 달려 있을 뿐, 언제 먹느냐는 큰 차이를 만들지 않는다고 결론지었다.

Another meta-analysis pooling 99 trials (6,582 people) found much the same. Only alternate-day fasting reduced weight slightly more than ordinary restriction (a mean of 1.29 kg); every other form of intermittent fasting, 16:8 included, showed no meaningful difference from an ordinary diet of the same total calories.

Where two hundred years of trial and error arrived was somewhere else entirely: a drug imitating a hormone the body already makes, and a verification procedure called the randomised controlled trial. Not one of the earlier methods ever passed through that gate.

        Since the first GLP-1 receptor agonist was approved as a diabetes treatment in 2005, the class has moved into the weight-loss market, and then from injection to tablet. Set out by US FDA approval record, it runs as follows.

        Tapeworms, cigarettes, vibrating belts, sleeping pills — one thing runs through the whole history of dieting. The wish to lose weight more easily and faster appeared with a new face in every era, and behind each was always a commercial interest and the aesthetic compulsion of the day.

        촌충이든, 담배든, 진동벨트든, 수면제든, 다이어트의 역사를 관통하는 것은 하나다. '더 쉽고 빠르게' 살을 빼고 싶다는 욕망은 시대마다 새로운 얼굴로 나타났지만, 그 이면에는 늘 상업적 이해관계와 당대의 미적 강박이 자리하고 있었다.

        GLP-1 drugs are clearly different from what came before in that their effect was verified by randomised controlled trials, and moving from injection to tablet lowered the threshold. Long-term safety, weight regain after stopping, access and cost are all still under discussion, though, and "easier to swallow" and "fine for anyone to take" are not the same statement. The seesaw has not stopped.

        GLP-1 약물은 앞선 사례들과 달리 무작위 대조 임상시험으로 효과가 검증됐다는 점에서 분명히 다르다. 주사에서 알약으로 넘어오며 문턱도 낮아졌다. 다만 장기 안전성, 투약 중단 후 체중 재증가, 접근성과 비용은 여전히 논의 중인 주제이고, '삼키기 쉬워졌다'는 것과 '아무나 먹어도 된다'는 것은 다른 말이다. 시소는 아직 멈추지 않았다.
